Who are our services for?
To obtain vision rehabilitation services, you must:
- have been diagnosed with an eye disease and have a significant and persistent disability that affects your activities of daily living;
- live in Laval, Montréal or the Montérégie region.
To register
- You must provide a recent vision report, completed by an eye care professional (ophthalmologist or optometrist), stating your diagnosis and your vision results.

Referral form
- It is important to send us any additional documents about your health condition (e.g., audiogram, occupational therapy assessment, medical information, cognitive assessment, etc.), as this will give us a better understanding of your general situation.
Forwarding your request
After receiving the vision report, an employee of the Service d’accueil-évaluation-orientation will contact you. They will review the problems you experience in your daily life and forward your request to the appropriate department.
Occupational rehabilitation
If your goal is to return to work, keep working, or achieve a professional objective, the occupational rehabilitation program at Institut Nazareth et Louis‑Braille can help you. To qualify, you must meet the same criteria mentioned above and provide a recent vision report completed by an eye care professional (ophthalmologist or optometrist). Referral form.
You have been accepted in the occupational rehabilitation program
To prepare for your first assessment meeting, you must fill out the Autoévaluation réflexive sur le travail. You must bring this document to your first meeting.
