Program objective
Depending on their deficiencies and limitations, disabled people generally use health and social services more often than the average citizen, the reason being that these services are essential to their social integration.
To promote access to services and enable disabled people to play an active role in society, the Transportation and Accommodation program for people with disabilities aims to provide them with financial assistance for transportation related to their intervention plan.
Eligibility criteria
To be eligible, individuals must meet the following criteria:
- live in Québec;
- live in the Montérégie region (Centre, East or West);
- not reside in an institution belonging to the health and social services network;
- meet the definition of a disabled person according to the Ministère de la Santé et des Services sociaux du Québec:
“Any person with a significant and persistent physical, intellectual, or mental deficiency leading to functional limitations and situations of disability.”
This definition includes people with chronic kidney disease who require hemodialysis while awaiting a kidney transplant.
Exclusions
The following individuals are not eligible for the program:
- People with physical health problems (e.g., cardiac or respiratory disease, diabetes, cancer, etc.).
- People whose transportation costs are reimbursed by other paying agents, for example:
- Société de l’assurance automobile du Québec (SAAQ);
- Commission des normes, de l’équité, de la santé et de la sécurité du travail (CNESST);
- Ministère du Travail, de l’Emploi et de la Solidarité sociale;
- People who receive income security (welfare assistance) are eligible. Transportation costs are reimbursed in addition to the contribution from the Ministère du Travail, de l’Emploi et de la Solidarité sociale.
- Department of Justice (detainees in a penitentiary);
- Department of Veterans Affairs;
- Department of National Defense (armed forces members, etc.);
- Health Canada (First Nations residing on a reserve, Inuit persons);
- Asylum seekers.
Transportation and eligible expenses
- Any travel over 50 kilometres (round-trip) between the place of residence and the network institution or the most appropriate place providing adaptation-rehabilitation, diagnostic, or treatment services related to the deficiencies recognized by the Régie de l’assurance maladie du Québec (RAMQ) and provided for in the person’s intervention plan.
- Any travel of 50 kilometres or less (round-trip) between the place of residence and the network institution or the most appropriate place providing adaptation-rehabilitation, diagnostic, or treatment services related to the deficiencies recognized by the RAMQ and provided for in the person’s intervention plan in the following situations:
- for preschool children (0-5 years) attending an early stimulation centre;
- for recurring treatments, such as hemodialysis;
- for medical treatments and adaptation-rehabilitation services that require intensive follow-up (at least twice a month for a minimum of 3 months).
- Meal expenses for the disabled person and their support person for medical treatments lasting longer than 4 hours (no receipts required).
- Parking fees (receipts required).
- Hotel accommodation when the required health services are located outside the Montréal metropolitan community (receipts required).
Registration process
To register for the Transportation and Accommodation program for people with disabilities, the section 1 to 4 of the " Application form/Intervention plan " must be filled out. It is important to clearly indicate the diagnosis of the person who will receive the financial assistance. This diagnosis must be certified by a health professional.
The section to 5 to 8 of the " Application form/Intervention plan " must be filled out by a health professional (social worker, human relations officer, doctor, nurse, etc.). The section 6 must contain the addresses of all locations visited by the user and all services received in relation to the diagnosis identified in the " Application form/Intervention plan ".
Once the form has been filled out and signed, it must be submitted as soon as possible to the Transportation and Accommodation program for people with disabilities at the CISSS de la MontérégieCentre. Here is the contact information:

Applications will be reviewed according to the program’s eligibility rules. The user will be informed of the outcome of their application.
Terms of reimbursement
Once their application has been accepted, the user can apply for reimbursement of eligible transportation costs. They will need a “Calendar of visits made for authorized transportation” form filled out by a health professional. By signing the form, they confirm the dates of visits made by the user to the healthcare institution identified in the section 6 of the " Application form/Intervention plan ". As needed, attach any required supporting documents. Send all forms and documents to the Transportation and Accommodation program for people with disabilities by email, fax, or mail, as mentioned above.
