CISSS de la Montérégie-Ouest

An assistive listening device (ALD) is a device that helps limit the impacts of hearing loss. It is designed to provide people with a hearing impairment more autonomy and independence. The comptoir des aides de suppléance à l’audition (CASA) (assistive listening device counter) is an accredited distributor authorized by the Régie de l'assurance maladie du Québec (RAMQ) to serve the needs of residents throughout the entire Montérégie region. 

Our services 

  • Distribute assistive listening devices to Montérégie residents
  • Repair or replace ALD when broken
  • Install visual and tactile alerting systems in the home

ALDs offered

  • Sound transmission aids: telephone amplifier, television listening system, FM system, personal amplifier
  • Text transmission aids: teletypewriter
  • Alert aids: visual or tactile alarm clock, ringing detector (door, telephone, fire, or baby crying) and visual or tactile detectors

 

The CASA service is free, but in order to have access, a user must meet the eligibility criteria of a paying agency (RAMQ, SAAQ, Veterans Affairs, CNESST). 

Request a service from CASA

To receive services from CASA, a user must consult an audiologist in order to obtain the documents required to assess the request:

For a RAMQ request

  1. The “Physical Disability Service Request” form OR the information required in sections 1,2,3,4,5 and 8*
  2. RAMQ form 3485 entitled Recommandation – Aide de suppléance à l’audition, filled out by an audiologist and dated within the past year. 
  3. A comprehensive needs assessment completed by an audiologist and dated within the past year, as recorded on form 3485 (“precision of need” section) or with a supplementary document (e.g., an audiology report).
  4. An audiogram issued by an audiologist and dated within the past year. 

For a request with another paying agency (CNESST/SAAQ/Veterans Affairs) 

  1. The “Physical Disability Service Request” form OR the information required in sections 1,2,3,4,5 and 8*
  2. An audiogram issued by an audiologist.
  3. Comprehensive needs assessment completed by an audiologist (audiology report).

Delivery and returns of ALDs for repair or replacement can be done from several facilities throughout the Montérégie region.
