To prevent the spread of HIV and hepatitis, injection drug users can obtain sterile injection equipment and a syringe disposal container by visiting their local CLSC during business hours.
For more information, visit: http://sante.gouv.qc.ca/en/repertoire-ressources/cami/.
To see a doctor with or without an appointment, you must call 450 299-2828 starting at 9:30 a.m. the same morning. You will not be able to see a doctor unless you call before showing up at the CLSC.
If your family doctor is at the CLSC, you can make an appointment with them for your regular check-up, treatments and preventive services. If necessary, your family doctor will refer you to other healthcare professionals.
To make an appointment, call your CLSC during the appointment centre business hours:
To obtain a summary of your medical record, a specific document from your medical record, or a medical certificate, contact the Medical Records department at CLSC de la Vallée-des-Forts.
You can consult a CLSC nurse for several health issues. With an appointment, they can give you advice and care following an injury or a surgery, or if you have a chronic disease.

To authenticate or renew your health insurance card, follow one of these two steps.
Go to a Société de l’assurance automobile du Québec (SAAQ) outlet with all the required documents, including the renewal notice for your health insurance card.
If less than 6 months have passed since the renewal date and you have an appointment with a doctor in the next few days:
Go to your local CLSC to have your application authenticated; you must bring all the documents required by the Régie de l’assurance maladie du Québec (RAMQ). A list of these documents is included with the renewal notice from the RAMQ. Among other things, you will need two pieces of ID and a recent photo. Visit the RAMQ website for more information about the photo specifications.
