You must have your health insurance card and your valid requisition form. Any requisition form that is not identified, not signed, incorrectly filled out, or not from Québec will be refused.
If your requisition is urgent and no time slot is available, you can call 1-833-699-2414 from 8 a.m. to 4 p.m., Monday to Friday, and we will assess your situation.
Colorectal cancer screening is now available at local points of service, without a referral.
If you are between the ages of 50 and 74 and meet the eligibility criteria, you can now book an appointment on the Clic Santé platform or by calling our appointment centre at 1-877-817-5279.

For all other age groups, consult a health professional for follow-up of your medical condition, with or without symptoms.
This screening test is for people aged 3 and over with symptoms suggestive of streptococcal A infection (fever, pain when swallowing, tenderness in the lymph nodes, and sudden onset of severe sore throat).
During your visit, the nurse will perform a clinical assessment, followed by a screening test, if required. If the result is positive, you will be given a liaison form to obtain the appropriate medication from your pharmacist.
