Reopening of the CLSC de Saint-Chrysostome

Please note that the youth vaccination clinics, held two (2) evenings per month, will continue to be held, by appointment only.

For blood tests, you must make an appointment via the Clic Santé website or call 1 833 699-2414 and select the option for the Haut-Saint-Laurent. It is necessary to make an appointment before coming to the CLSC.

Do you feel like there’s no solution to your problems? Do you feel sad, upset, drained, and mentally or even physically overwhelmed? Are you having a hard time sleeping and thinking about putting an end your suffering?

Rest assured that help is out there and that there are people willing to listen.

Services are available from Monday to Friday, 8 a.m. to 4 p.m., at the CLSC de Saint-Chrysostome.

Our partners are also there for you!

You can call the crisis and suicide prevention centres La Maison sous les arbres at 450-699-5935 (Châteauguay) or Le Tournant at 450-371-2820 (Suroît) at any time, 24 hours a day, 7 days a week.

No matter what your age, you can call the provincial suicide prevention hotline at any time at 1-866-APPELLE (277-3553). They will refer you to the appropriate resources, services and professionals.

Young people who need someone to talk to can also get help by calling Tel‑jeunes at 1-800-263-2266 or Kids Help Phone at 1-800-668-6868.

Contact the psychosocial intake department at your local CLSC, which will analyze your needs and refer you to the relevant services.

What is an intellectual disability?

Intellectual disability is a disability characterized by significant limitations both in intellectual functioning and in adaptive behaviour, which covers a range of everyday social and practical skills. The impairments appear before age 18.

What is autism spectrum disorder?

Autism spectrum disorder is a neurodevelopmental disorder that significantly affects social interaction, communication and behaviour. It also manifests in the form of atypical behaviours and the development of narrow or extreme interests in specific topics. People with ASD are a highly heterogeneous group; depending on the type of impairment, they will need different types of support.

To obtain a summary of your medical record or a specific document from your medical record at one of the CLSCs on the Haut-Saint-Laurent territory, you must:

  1. Submit a signed written request using one of the forms below.
  2. Send your request to our medical records department:

*Please note that you can also submit a written request at the CLSC’s front desk.

IMPORTANT: Make sure to fill out all fields, and sign and date the form.

All requests concerning a person under the age of 14 must be signed by the mother OR the father OR the legal guardian. In compliance with the Act respecting Access to documents held by public bodies and the Protection of personal information, a fee is payable in advance for certain services.

Types of problems managed by the general social services team

  • Adjustment problems or disorders related to life events or stages (e.g., bereavement, illness, separation, financial, family or marital problems, etc.)
  • Crisis situation
  • Symptoms of anxiety, depression and/or obsessive-compulsive disorder
  • Relational problems (the intervention targets the psychosocial factors that impact the person’s life)
  • Behavioural problems and their consequences (e.g., aggression, addiction, etc.)
  • Stable mental health disorders with occasional psychosocial needs
  • Eating disorders
  • Conjugal violence, sexual assault victims, etc.

Description

If you have a driver’s license

Go to a Régie de l'assurance automobile du Québec(RAMQ) outlet with all the required documents, including the renewal notice for your health insurance card.

If you don’t have a driver’s license

If the renewal period expired less than six months ago and you have an appointment with a doctor in the next few days, go to your local CLSC to have your application authenticated; you must bring two pieces of identification and a recent photo that meets the RAMQ criteria.

  • colour photo taken in the past six months (date stamped on the back)
  • white background
  • front view of head and top of shoulders (head must be uncovered)
  • 43 mm x 54 mm (1 11/16 inches x 2 1/8 inches)

Description

We offer vaccination services for children aged 2 months to 5 years. Our vaccines cover diphtheria, pertussis, tetanus, poliomyelitis, serious Hib infections, hepatitis B, rotavirus, pneumococcus, measles, rubella, mumps, type C meningitis, influenza and chickenpox.

Consult the youth clinic

  • Are you between the ages of 12 and 24?
  • Do you have questions about sexuality, birth control, your love life, or your relationships with your friends or parents?
  • Are you pregnant?
  • Do you feel alone or self-conscious, and want to talk to someone about it?
  • Do you have thoughts of suicide?

Help and support are never very far away.

Free and confidential services
Parental consent is required for children under age 14.

At your school
There are several people who can help you at your school, including the nurse, social worker, specialized educator, psychologist, and remedial teacher, among others.

In high schools, the youth clinic is run by the school nurse.

Youth centres
There are several youth centres in your region. You’ll meet other young people there who are just like you—new friends you can talk to and confide in. You’ll also find people who are always willing to listen if you feel the need to open up.
