Description

With a referral from a doctor or a healthcare professional, the day hospital provides specialized services to seniors with one or more unstable physical, psychological or social problems. Certain services are also available for family members and health and social services workers.

Users typically require multidisciplinary interventions, e.g., diagnostic assessment, treatment (including rehabilitation), discharge, referrals, training and support.

Registration

Your referral to the day hospital must be filled out by a doctor or by any other healthcare professional, with the treating physician’s approval.

How it works

After the user is admitted, a multidisciplinary team assesses their health condition in order to:

  • agree on an intervention plan with them;
  • set treatment goals adapted to the user’s needs;
  • request certain diagnostic tests, as needed;
  • regularly assess the user’s progress and adjust the intervention plan;
  • send the treating physician results and recommendations.

The frequency and duration of appointments varies depending on the user’s needs.

Description

The Medical Records Service allows you to obtain or forward a summary, a specific item or a certificate of service/residence related to a person's medical record, all in compliance with the Loi sur les renseignements de santé et de services sociaux (R-22.1).

To be admissible, all requests must be submitted in writing, as follows, depending on the type of applicant:

Person 14 years of age or over or parental authority of a person under 14 years of age 

Complete form AH-216 or submit a written request. This must include the following information

  1. Surname and first name of the person concerned by the request
  2. Date of birth of the person concerned by the request
  3. Nature and dates of information requested
  4. Applicant's name and address
  5. Applicant's signature
  6. Date of signature

Legal representative of a living person 

Complete form AH-216 or submit a written request. This must include the following information

  1. Surname and first name of the person concerned by the request
  2. Date of birth of the person concerned by the request
  3. Nature and dates of information requested
  4. Applicant's name and address
  5. Applicant's signature
  6. Date of signature

Please enclose with the application documents proving the status of legal representative
