Services offered

The Centre de réadaptation en dépendance (CRD) offers services to provide detoxification, rehabilitation, and social reintegration services to people with addiction problems related to :

alcohol;
drugs (including prescription medication);
gambling and compulsive buying;
problematic Internet use.

It also offers services to the friends and family members of clients, whether or not they are registered for the CRD’s services.

Our team of professionals can help you to:

Reduce the negative consequences of your substance, gambling, compulsive buying, or Internet habits;
Adopt responsible behaviours for dealing with them;
Acquire new skills for coping with daily challenges.

Description

To obtain a summary of your medical record or a specific document from your medical record, you must send a duly signed written request to our Medical Records department by fax at 450-443-0522, by email at archives.crdlevirage16@ssss.gouv.qc.ca, or by mail to the following address:

The request must contain the following information:

your family name and first name;
your date of birth;
your medical record number (if available);
your complete address;
your telephone number;
the facility where you received care;
the nature of your request (clearly indicate which documents you require);
the date by which you need the information.

Then, sign the request and indicate the date of the signature (recent). In compliance with the Act respecting access to information, a fee is payable in advance for certain services.
