This service is available to users of all ages with functional disabilities who require technical aids for positioning and mobility.
Through the use of orthotics and mobility aids, the service helps people with physical disabilities overcome issues that affect their day-to-day functioning, to avoid them being at a disadvantage in their home, school, work or community environment.
The CISSS de la Montérégie-Ouest has an exclusive agreement with the Régie de l’assurance maladie du Québec (RAMQ) in the Montérégie region.
The services provided include:
- the assessment, design, production and dispensing of various technical aids;
- teaching on the optimal use of mobility aids;
- the adjustment and repair of aids provided;
- emergency assistance.
Examples of technical aids
Mobility: folding/rigid manual wheelchair, motorized wheelchair, tilt wheelchair, special-needs stroller, and standing frame;
Positioning: special-needs back and seat cushions, positioning aids, tilt‑in‑space wheelchair, etc.;
Walking: standard walker, speciality walker, quadpod cane, single cane (with leg orthotics), and crutches. Orthotics: Legs, arms, corsets, and orthopedic shoes.
Documents required to send an assessment request to TAS
Positioning and mobility aids
- Technical aids service referral form
- Medical certificate confirming a significant and permanent physical impairment
- Medical report confirming the diagnosis(es) OR
- Enter the source(s) of the diagnosis(es) on the TAS referral form (type of medical document, location, name of physician, date of medical certificate)
- Any relevant document needed to confirm musculoskeletal involvement (e.g., x-rays, results of neurological tests)
- RAMQ form 3841 - Attestation du besoin d'une aide à la locomotion ou à la posture, filled out by an occupational therapist, physiotherapist, or physical rehabilitation therapist (if the user is followed by one of these professionals). The referring therapist must fill out sections 1-11 of the functional assessment. If the referral is for a positioning aid only, form 3841 is not required.
A medical prescription is required for a mobility/posture/walking aid in the following situations :
- Device under special consideration (SC) – without order
- Component of the SC device
- Device less than 5 years old to be replaced due to a change in the user’s condition
- Pediatric device (stroller, personal assistance service device (PASD), wheelchair, tilt wheelchair, walker)

- Technical aids service referral form
- Medical prescription from a specialist recognized by the Régie de l’assurance maladie du Québec (RAMQ) or request for a consultation in physiatry (general practitioner), as required
A medical prescription is required for an orthopedic corset (covered by the RAMQ).
Specialists recognized by the Régie de l’assurance maladie du Québec (RAMQ) include: orthopedic surgeon, physiatrist, rheumatologist, neurologist, neurosurgeon, and geriatrician. A general practitioner with prescribing privileges in the area of physical disabilities may also write the prescription.
All documents must be submitted to your local TAS point of service (Châteauguay, Saint-Hubert or Saint-Hyacinthe).
